Immigration Medical Exam
One essential step is the immigration medical exam, conducted by a USCIS-approved Civil Surgeon. This exam ensures you meet permanent residency's health and vaccination standards. Here's what to expect:
- A comprehensive physical examination
- Review of your medical history and vaccination records
- Lab tests to check for infectious diseases
The results of this exam are vital for your Green Card eligibility. It's a critical milestone on your path to becoming a permanent resident in the U.S.
What to Bring
- Valid passport or other government-issued photo identification such as state ID, driver’s license, travel permit, or work permit.
- (Alternative proof of identity may be accepted for children under 14 years of age that shows name, date and place of birth, parents’ full names and other identifying information).
- Immunization / Vaccination records (English Translation), including proof of COVID-19 vaccination, unless it is not available for you (e.g if you are under 6 Month old).
- Alien Registration number if you have one (starts with an “A”).
- The required fee for the exam – Cash, Debit and Credit Cards only.
- An Interpreter who can speak, read and write in English if needed.
- List of medications (if you are being treated for a chronic medical condition or taking medications on a regular basis) or a letter from your regular doctor outlining the treatment plan for any health problems you have.
- Tuberculosis certificate from your doctor (if you’ve had a previous positive skin test for tuberculosis) proving that you were adequately treated.
- Certificate of clearance signed by a doctor or public health official, proving that you were adequately treated (if you have had syphilis).
- If you have a history of harmful or violent behavior resulting in injury to people or animals, information that will allow the doctor to determine whether the behavior was related to a psychiatric or medical problem, or to drug or alcohol use.
- If you have been treated or hospitalized for psychiatric or mental illness, or alcohol or drug abuse, written certification including the diagnosis, length of treatment, and your prognosis.
Cost of Exam
We do not accept medical insurance for all services required for the 693 Immigration form. What is included in the cost of Exam
- Basic Immigration Medical Physical Examination
- Quantiferon (Tuberculosis) Test
- Gonorrhea Test
- Syphilis Test
- Vaccination status verification
- Copies of your laboratory results
- Certified and Sealed I-693 Form
What is NOT included in the cost of Exam
Additional tests listed below are not included in the price of the exam but are offered for an additional fee. This will add to the cost of the exam.
- Vaccine titers

- Chest X-rays

After The Exam
After the examination, Dr. Wilmot-DeSouza will complete, sign, and seal the I-693 Form and include any supporting documents in an envelope that reads DO NOT OPEN. You must submit the sealed envelope to USCIS as directed in the Form I-693 instructions.
